Medical Case Management

Case Management at New Mexico AIDS Services is an integrated, comprehensive, client-centered system of care designed to address the needs of individuals living with HIV or AIDS.  NMAS Case Managers determine and review eligibility for services, coordinate client services, and provide referrals.

For eligible clients, the services that can be accessed through the NMAS case manager include health & medical services, assisted living, food bank access, transportation assistance, support groups and other support services. Case Management services for individuals applying for New Mexico’s AIDS Waiver program are also available.

Eligibility requirements: Individual must be HIV+ and under 400% of Federal Poverty Level guidelines.

Family Case Management

Family Case Management at NMAS is an integrated, comprehensive, client-centered system of care designed to address the needs of individuals and families infected or affected with HIV/AIDS. Family Case Managers determine and review eligibility for services, coordinate client services, and provide referrals.

Clients eligible for Family Case Management services include transgendered persons, youth (age 24 and under), and adults with dependent children. Exceeding the 400% of Federal Poverty Guidelines does not make clients ineligible for NMAS Family Case Management Services. Affected family members (spouses and/or dependent children) may also access services.
